 Do you want an affordable dentist, but have no dental insurance? Well, now it's easier than ever to get the dental care you want or need with the Life Long Care Plan.
What is the Life Long Care Plan?
Our goal is to help you have the beautiful, healthy smile that you want and deserve. There are many reasons why people stay away from the dentist, but one comment we hear frequently is, "I can't go to the dentist because I don't have insurance."
We are excited to now offer the Life Long Care Plan for you, your friends, family, or co-workers who have no dental benefits. When you sign up for membership, you will simply pay an annual enrollment fee, and then you will have access to the following dental benefits, all included for you as part of the cost of enrollment. This saves you money overall.
Dental Services included in the Plan:

Annual Dental Exam, necessary x-rays, and a dental cleaning (valued at $400)

You will save at least $270 on your first two visits!
What's the Cost?
The Annual Enrollment Fee is only $350. Each additional family member is $230 for the same benefits. Qualified family members include spouses and children up to age 18.

Terms & Conditions: 1.) Enrollment fees are non-refundable once any treatment is performed, including the first cleaning. 2.) If you have periodontal disease and require either 3 or 4-month periodontal maintenance appointments; these services will be partially covered. The value of the two simple cleanings will be applied to the cost of the first two periodontal maintenance cleanings. The third and fourth periodontal cleanings will be subject to a 10% discount. 3.) Payment in full is due on the date of service. 4.) Enrollment fee subject to change at any time. By filling out the enrollment form, you are choosing to enroll in the Life Long Care Plan. Your enrollment will expire 1 year from the date enrolled. Please indicate if you would like the plan to renew automatically. |
